What to Bring to Your Appointment
Arrive at least 15 minutes early and bring:
- Government-issued ID (e.g., driver’s license)
- Insurance card
For a thorough assessment, please also bring:
- Test results
- X-rays
- Mammograms
- Ultrasounds
- Any other clinical documents
Save Time with Pre-Appointment Forms
Fill out these forms before your visit to speed up the check-in process:
- Registration Form
- Medical Release Form
- Notice of Privacy Practices (for your reference)
Download, complete, and email the forms to [email protected], or bring them to your appointment.
Cancellation Policy
If you need to cancel, please provide 24 hours’ notice to allow us to assist other patients.
Keeping Your Doctors in the Loop
We’ll update your primary care or referring doctor on your condition and treatment progress.
